Cheat/Hack Reporting Rules

i am new, just joined actually and here is my first question(of many i am sure)

What exactly do you need in the reporting of a cheat to get them banned, please just let me know what you need to see in the videos, a set of rules to follow. Hope to hear from you soon

This would be an example of one i have done for the BF Heroes folks

Level of Stealth (If Any):-5/5
My Hero Profile:-My Profile Link
Suspect Name:-[Spectre]
Suspect Profile:-Profile Link

Oh one more thing, if a cheat is identified is it possible to ban all of his/her hero names. it only seems fair

What we Need
-Name of offending Hero and/or username (duh lol)
-Video Proof, the more the better. [Either upload to here or have it uploaded on a viewing site]
-If commando, post your stealth level.

What we would like to have:
-Name of i3D.net server/which one of which the event happened (Even though it is a global ban)
